The information you provide on this form is confidential and is used only to assist the Volunteer Coordinator in finding a suitable placement for you.

This policy is part of our commitment to ensure the church provides a safe environment for all. Since November 1993, the Council of General Synod has required that all staff members and volunteers indicate their adherence to this policy.
